How to fill ‘Entry Form to Switzerland’
Due to the coronavirus epidemic, special conditions are applied in Switzerland, as in most countries.
The entry form to Switzerland is an entry form administered by the Federal Office of Public Health (FOPH). Anyone entering the country must fill out this form. Filling out the form is mandatory for those who will enter the country by plane. Passengers using the country as a transit must also fill in.
Filling the entry form to Switzerland

1. When filling out the form, first choose your entry method. This part has been made easy to understand with pictures of airplanes and ferries. If it’s an airplane, select the airplane. Then select your arrival day in Switzerland from the “Date of arrival” section.
2. From the “Place of departure” section, select the airport you boarded from. For example, if you boarded from Sabiha Gökçen Airport, you should choose “Istanbul (SAW)”. Select the country you come from from the Country/Region section. Fill in the flight information, if you don’t know the seat number, you can make it up (like 11A) because they don’t check this part.
3. You must fill in the personal information in the Person section. First select gender: female, male, other. Write your name in “First name” and your surname in “Last name”. Select your birthday in the “Date of birth” section, enter your passport number or Swiss ID number in the “Passport / ID Card Number” section. Enter your phone number in the “Mobile number” field.
4. Enter your Swiss address in the “Permanent residential address” field. If you are going to be in Switzerland for a short time, enter your address in the country where you live. If you live in another country, enter the temporary address where you will stay in Switzerland in “Address of stay in Switzerland”. If this is not the case, select the same address with the “Same as permanent residental address” option and pass.
5. If you have a child under the age of 16 or a legal heir with a disability, enter below. Then go ahead and check. If everything is OK, it will ask you to enter your email address and click on the link that comes to your email address. Download the received QR code document to your phone or use the QR code in the email.
Requirements for entry to Switzerland
On the other hand, people coming to Switzerland from countries and regions where the risk of infection is high must go into quarantine. Turkey is not yet on the quarantine list.
Upon arrival in Switzerland, PCR testing is also requested from those over 16 years of age. If you have been vaccinated or have recovered from COVID-19 in the past 6 months, you may not need to submit testing. Otherwise you will need to provide evidence of a negative PCR test (not older than 72 hours) or rapid antigen testing (not older than 48 hours). Vaccines from Pfizer/BioNTech, Moderna, AstraZeneca, Janssen / Johnson & Johnson, Sinopharm / BIBP, Sinovac are accepted for vaccines.
